Our Rehabilitation Assistants provide one-to-one support tailored to your individual goals — delivered in your home, your community, or alongside activities that are important to you.
We work alongside your existing therapy team where appropriate, or we can help you get started if you are looking for rehabilitation support for the first time.
All support is overseen by our HCPC registered neuro specialist physiotherapist, ensuring that your rehabilitation plan is clinically informed and delivered safely.

It's about feeling confident to get out into your community again, staying connected to the people and activities that matter to you, and feeling able to manage everyday life at home — at your own pace, with someone alongside you who understands what that takes.
Our Approach
Rather than seeing support sessions as simply completing tasks, our Rehabilitation Assistants are trained to recognise the rehabilitation opportunities that naturally exist within everyday activities.
Using shopping as an example, depending on your goals, a session might include opportunities for:

Initial Consultation
We begin with a detailed discussion about your medical history, current abilities, goals, and any recommendations from healthcare professionals.
Individualised Support Plan
A personalised rehabilitation support plan is created to reflect your needs, goals and long-term objectives — overseen by our HCPC registered physiotherapist.
Goal-Focused Sessions
Sessions focus on supporting your rehabilitation goals, building functional independence, confidence and community participation.
Ongoing Review
Progress is continuously monitored and reported, with regular review of goals and adjustments to support as your needs evolve.
